To
receive home health care services the patient must:
- Have
a health concern requiring home health care.
- Have
written orders from home health care from an attending physician,
who will continue to provide medical supervision.
- Understand
and agree to the home health care plan (along with the family).
- Reside
within the geographic services area for which the Visiting Nurse
Association of Central Connecticut, Inc. is licensed.
Services
are available to all persons who meet eligibility requirements regardless
of age, sex, race, religious affiliation, color, national origin,
handicap or financial status. Patients remain under our care until
they no longer require skilled services or they no longer meet the
eligibility requirements. Service
